Athletic Forms and Fees
In order for a student to practice or participate in Ursuline Academy Athletics, the student must meet all eligibility requirements as listed in the handbook and submit all paperwork to their head coach prior to the start of the season. Athletic fees will be billed to a student's Blackbaud account the month as rosters are finalized. Failure to pay the athletic fee may affect eligibility. First-year participants in a high school sport must also submit a copy of their birth certificate. Please download the following forms:
High School
2025-2026 Athletic Fee: $150 per sport; $100 for any additional sport

Concussion Packet
Parents, students and coaches must read and sign their respective acknowledgment sheet. Forms must returned to your coach.
Middle School
2025-2026 Athletic Fee: $100 per sport; $50 for any additional sport
